Who We Are
Primary Care Networks (PCNs) is a NHS initiative to provide at scale primary care services closer to patients, and working closely with other health and social care partners to provide seamless care to our registered population.

Our PCN is made up of 8 practices which have teamed up to start providing patients with a wider range of health and wellbeing services, specifically geared towards the needs of their local areas.
We believe in Unity in diversity
Our allied staff are made up of clinical pharmacists, physician associate, physiotherapist , podiatrist, social prescriber, health & wellbeing coach, Care Co-ordinator and Occupational therapist . A dietitian, paramedic and psychotherapist will be joining our team in the near future.
Our stakeholders are: Local council , PHE , NELFT, CCG, PPG, BHURT, St. Francis Hospice, Charity organisations such as Age UK , Cancer UK, McMillan and many more.
